Our team looks forward to meeting you and getting to know you, but first, there’s some vital medical and dental information we need to gather before we can provide the appropriate treatment for your needs.

Visit our forms page, and you’ll find new patient registration forms for adults and children. Please download, print, and complete these forms before your first visit. If you are unable to finish them before your appointment, please plan to arrive 15 minutes before your scheduled time and fill them out here.

Please also bring your photo ID and any insurance cards you plan to use.

When You Arrive

Our amazing dental team loves meeting new patients and will greet you by name and welcome you warmly to our practice. During your brief wait, relax and enjoy our comfortable reception area!

Your Dental Cleaning & Exam

We will show you back to your treatment room promptly and get you settled comfortably. Unless you have a dental concern that you would like to speak to one of our dentists about, your first appointment will be with one of our friendly dental hygienists.

During your dental cleaning, your hygienist looks for signs of gum disease and removes plaque and tartar from your teeth. She notes any areas of concern and gives you tips on improving your oral hygiene at home.

If you have questions about brushing and flossing or any other oral health issues, this is the perfect time to ask! Patient education is an essential part of what we do, and our dental team is always more than happy to help!

After your examination, we’ll explain the dental problems we need to address and the treatments we’ll use to get your oral health back on track. Our dental care team will review insurance and financing options based on the procedure we recommend and schedule future appointments for you as needed.
